Implant Selection
Perhaps the most important factor in the look and feel of your new breasts is the size, shape and material of the implants you select.
Silicone Gel Filled Implants
The silicone gel filled implants available today are vastly improved over earlier models, and are becoming increasingly popular. Current silicone implants are made of a cohesive gel, which is not as likely to leak if the implant is punctured or torn. Silicone has been used in medical devices for decades, and has proven safe and effective in exhaustive studies and by the FDA.
The greatest advantage of silicone gel filled implants is the more natural looking and feeling results that they offer. There is less incidence of rippling or wrinkling than with saline filled implants, and no immediate loss of volume in the case of rupture. Disadvantages of silicone implants include a longer incision, as well as slightly increased cost due to increased manufacturing expenses.
Saline Filled Implants
Saline filled implants remain a popular choice for women who undergo breast augmentation in New Hampshire. These implants are filled with a medical grade saltwater solution that is harmlessly absorbed by the body in the case of rupture or leakage. They are less expensive than silicone implants, require a smaller incision, and offer Dr. Sepic more options for placement of the incision. Since these implants are not pre-filled, Dr. Sepic also can adjust the fill level to accommodate for breast asymmetry.
One of the disadvantages of saline filled implants is the immediate loss of volume that occurs if the implant ruptures. Another concern is the wrinkling or rippling that sometimes can be associated with these implants, particularly in women with thin skin or very little breast tissue. In general, breasts augmented with saline implants do not look and feel as natural as those with silicone implants, which can be a deciding factor for some patients.
Incision Options
Dr. Sepic offers a variety of incision options for his Portsmouth breast augmentation patients, depending on the implant type and your particular anatomy and personal preference.
Periareolar
This incision runs along the lower border of the areola, or more darkly pigmented circle of skin around the nipple. This allows fairly direct access to the interior breast anatomy for placement and adjustment of the implant, and leaves almost undetectable scars.
Inframammary
This is the most commonly used approach for both saline and silicone implants. This well-concealed incision runs along the underside of the breast where it meets the rib cage. It allows the best access to the interior breast anatomy for optimal placement and adjustment of the implants.
Transaxillary
Also called the armpit incision, this option is generally available only to patients who choose saline filled implants. The incision is made in the folds of skin in the armpit, so there is no visible scarring on the breast. This approach requires a skilled surgeon, as the unfilled implant must be carefully tunneled into place before being filled.
Implant Placement

- Most patients choose submuscular or subpectoral placement, which involves placing the implant under the chest (pectoralis) muscle. Due to the deeper incision and stretching of the muscles, this approach has a slightly longer and less comfortable recovery. However, this placement typically makes for a more natural looking and feeling result. Dr. Sepic performs a majority of breast augmentations using submuscular implant placement.
- Subglandular placement of the implant puts it directly behind the breast tissue and above the chest muscle. This makes for a quicker, more comfortable recovery, but this approach is not the best choice for many women. Implants tend to be more visible in this placement, particularly in women with thin skin or very little breast tissue. This placement may also cause more interference with subsequent mammograms than submuscular placement.
